Breast Health Diagnostic Services
Susan G. Komen San Diego and Barbells for Boobs provided funding for necessary diagnostic services for uninsured and underinsured men and women living in San Diego, Imperial, and Riverside counties that required breast health diagnostic services who did not qualify for the State’s Every Woman Counts program. Diagnostic services included mammography, surgical consultations, breast biopsies, ultrasounds and ancillary services. Individuals in need of diagnostic services must be referred by their community health center or an Every Woman Counts provider.

SOCIAL DETERMINANTS OF HEALTH (SDOH)
PLANNING FOR THE SPREAD OF PROTOCOL FOR RESPONDING TO AND ADDRESSING PATIENTS’ ASSETS, RISKS, AND EXPERIENCES (PRAPARE) IN CALIFORNIA
Social determinants of health are the conditions in which people are born, live, work, and age that affect their health. Health starts in our homes, schools, workplaces, neighborhoods, and communities. Health is also determined in part by access to social and economic opportunities; the resources and supports available in our homes, neighborhoods, and communities; the quality of schooling; the safety of workplaces; the cleanliness of water, food, and air; and the nature of social interactions and relationships.

ALL Heart
All HEART was a program targeted towards reducing heart attacks and strokes through the identification and treatment of diabetes and hypertension using evidence based guidelines among the safety net population in community health centers in Southern California. Kaiser Permanente’s ALL initiative (Aspirin, Lisinopril and Lipid-lowering medication) was implemented to reduce heart attacks and strokes by aggressively enrolling patients with heart disease or patients who are at least 55 years old with diabetes, in a therapeutic program that included the use of these three medications. Patients based in community health centers in Southern California were the program’s focus. In partnership with Kaiser Permanente, HQP worked with several health centers and expanded ALL HEART to additional health centers. While the focus had been on patients in community health centers with diabetes, in the 2016-2017 year funding cycle, HQP expanded focus to include hypertension. ALL HEART identified health center patients ages 50 and over with diabetes and patients age 18 and over with hypertension.

Home Blood Pressure Monitoring: Reimagined Care Challenge 2016
This program sought to engage patients in assisting with management of their hypertension by utilizing an at home blood pressure monitoring program. Patient materials and program activities were developed based on the outcomes of separate co-design sessions with patients with hypertension and care teams within community health centers. A pilot program was conducted using the developed materials which were well received by patients and community health center staff.

Million Hearts
Million Hearts established a two-year cooperative agreement with the National Association of Community Health Centers (NACHC) to pilot the Million Hearts quality improvement strategy and tools to improve the detection and control of high blood pressure in community health centers that serve populations disproportionately burdened by hypertension. NACHC recruited 11 federally qualified health centers nationwide to participate in this program including two Health Center Partner member health centers, La Maestra Community Health Centers and Neighborhood Healthcare.
